Care Management

Concerned about safety?

Perhaps Mom recently fell and broke a hip or maybe Dad is having trouble sticking to his doctor's recommendations. Does your loved one seem to be isolated? Losing weight? Having difficulty with remembering important activities or events?

Want to stay at home?

We at LifeCare Solutions can assist your family. We can provide you and your loved one with the support needed to stay at home, and suggest alternatives, as needed, to make sure your parent is safe and comfortable.

How we Contribute

How We Contribute

Peace of Mind

As a family member, you want to be sure your loved one has a credible plan in place. Our collaborative approach makes life easier for you. You can spend more time being a son or daughter and less worrying if your parent is safe.

Objective Assessment

As Aging Life Care™ Managers, we give you expert advice outlining the current situation and making recommendations for needed support services now, and in the foreseeable future.

Prevent and Manage Crises

Of course, we prefer to be brought in before there's a crisis. But if one occurs - a fall, a hospitalization, a stroke or a wandering incident - we can jump in and handle the medical issues. We can also support you, and your relative if changes in living situation are required.

Serve as an Advocate

It's a complicated maze. Without a knowledgeable advocate, the patient's voice is frequently drowned out. Let us help your loved one determine his or her goals of care, and then work with the medical system to make sure treatment decisions are made that support those goals.

Preserve your Relative’s Independence

One of the primary challenges of those needing care is retaining as much control as is appropriate, and then learning to let go. Everyone deserves the "dignity of risk," thus we support autonomy and self-determination for as long as it's realistically possible.

Recommend Resources

Based on your family's preferences and financial situation, we can make recommendations for support services that are the best fit. We all want your loved one to be satisfied with the decisions made. We put our experience to work for you so the odds favor a successful transition.

SUCCESS STORY

MEET SALLY & BILL

“Sally and Bill lived at home while all of the kids lived out of state. Sally has dementia and Bill was caring for her. While Bill was helping Sally, he fell, leaving him in the hospital. Their son was not sure what to do to help so he called LCS. LCS put caregivers in the home and created a schedule for Sally. LCS took over medication management, doctor appointments and care plans for her. When Bill came home, they did the same for him. LCS were able to help strengthen both parents and help them find the right service and equipment for their wellness, as well as monitor and correct their issues.

LCS helped their family navigate and manage long term goals and planned for both Bill and Sally on an individual basis while giving the family updates and keeping them at the level of involvement that is emotionally best for them. This maximized the adult children and spouses’ strength and contributions to each parent's journey - everyone got to be part of the team.”
